CBD vs THC for anxiety: CBD and balanced flower groups improved far more than THC-only
Human naturalistic study (no placebo) · meta-analyses mixed AI-graded ⓘ
AI-graded: the grade and plain-words summary were written by AI from the cited studies; PubMed and DOI links were checked automatically. Not reviewed by a doctor. Always check the source.
345 adults using cannabis for anxiety were randomly assigned THC, CBD or both for 30 days. In the flower groups, anxiety dropped 7.8% with 24% THC, 39.5% with 12% THC / 12% CBD, and 34.8% with 24% CBD.
The catch, graded honestly: no placebo, not blinded, and people used as much as they liked. Anxiety fell in every group. The authors call it associational, not proof.
The bigger picture is mixed. A 2026 meta-analysis (Psychiatry Research) found cannabinoids lowered anxiety symptoms in trials, with very low certainty. A 2026 meta-analysis of 54 RCTs (Lancet Psychiatry) found no significant effect on anxiety outcomes.
Mood and depression: neither 2026 review found a benefit, and Lancet Psychiatry found no RCTs that treated depression as the main target. In mice, CBD + β-caryophyllene (a peppery terpene) eased pain and depression-like behavior (Dec 2025). Mice only.
High THC can raise anxiety.
Typical picks: CBD-dominant or balanced. ACDC, Ringo's Gift (high CBD); Harlequin, Cannatonic (balanced). Check the COA.

- Sleep: CBN and sleep: a high dose helped people fall asleep faster, but did not cut night waking
- Chronic pain: THC:CBD ratio and chronic pain: balanced or high-THC gave small drops; low-THC/high-CBD may not help
- Migraine: THC + CBD flower vs migraine: 67% got relief at 2 hours, vs 47% on placebo
- Inflammation / arthritis: β-caryophyllene and arthritis: it cut swelling in rats, while a THC pill didn't beat placebo for knee pain
- Appetite: CBD and appetite: healthy adults ate 193 kcal more after CBD, without feeling hungrier
